Free Fat transfer (FFT), sometimes called liposuction fat transfer (LFT) is a surgical method of penis enlargement that is advertised readily across the Internet and other media. A simple Google search of "penis enlargement surgery" and you will most likely come across a few clinics that are offering this procedure. Simply put, the FFT surgeon will lipo fat cells from a region of the body and inject this liquid adipose below the skin of the penis. This FFT material is not sutured across the tunica, and is freely dispersed around the corporal penis body. When transferred tissue is not sutured across living tissue, there will be no support for life and longevity of the transferred tissue. The introduction of plasma, and bodily proteins can not possibly maintain tissue life on their own, the introduction of adherence to a supportive living tissue is essential for tissue permanency.

No harm no foul, right? The FFT absorbs and men are left with what they started with. If it were that simple, this web page would not exist. Often the FFT absorbs unevenly, leaving the penis lumpy, uneven, and down right unsightly (see figure 1A). Intervention is often required, or desired by the unsatisfied patient. Often, the FFT surgeon with explain to the patient that absorption is very rare, and provide self proclaimed statistics to back their procedure. Our favorite, "you didn't follow post-op protocol," or "we will repeat the procedure, more will take next time." Typically, lumps are not removed, but more FFT is injected to fill around the area.

After removing the scattered FFT material, it is replaced with permanent dermal, or Alloderm tissue grafts. The penis will feel very hard in the erect state, much like their natural erection hardness.
