To begin with you must get to 100% erection level or as close to that as possible. Next you should clamp off the base of your penis using an OK grip with one hand. You can use either the standard (palm facing upward) or reverse (palm facing downward) OK grip. Once you have trapped the blood in your penis release your grip very slightly and at the same time kegel in as much extra blood as you can.
* Then re-tighten your grip again to trap the extra blood.
Now that you have your penis filled with blood and your hand clamping it off at the base, the idea is to slide your clamping hand up your shaft slightly, maybe an inch, in order to increase the pressure within your penis. You should use lube for this exercise. Now take your free hand and place your palm on top of your glans. Start pushing down toward your clamping hand without letting your OK grip loosen or letting you hand slide back down the shaft. At the same time as you are pushing down on your glans push your clamping hand upward, pushing your hands together. This should increase the amount of pressure in your penis considerably. Hold this position maintaining the pressure between the hands.
As the exercise goes on some blood will inevitably slip past your clamping hand but keep your grip tight and keep the upward pressure from your clamping hand going as well as the downward pressure from your other hand. This should ensure a good amount of expansion throughout each rep.
Instead of using one hand to press down on their glans, some people push their penis into a wall. That works the same as the hand that would be on the glans by providing downward pressure.
Hold this position for between 10 seconds and 1 minute. You can hold these for longer as you become more experienced and DLD likes to see this exercise as sexual or masterbatory. That is a good way of looking at it and he holds his reps for 2-3 minutes.
This is one repetition.
Do ten or more of these consecutively for a good all-round girth workout. Swap hands after each repetition.
These are possibly the best exercise for girth in general so as always, be careful since they are can get VERY intense.
* At this point some people prefer to squeeze some blood out of their glans in order to give them enough room in the penis to work with later in the exercise.
